Congratulations on your upcoming graduation and best wishes as you begin the next step in your medical education. You are wise to consider continuing your disability coverage in the AMA sponsored Med Plus Advantage Program. If you choose to do so you are protecting your future with a quality insurance product that is tailor-made to meet the unique needs of physicians. For your convenience, a Resident Continuee Enrollment form may be downloaded and printed by clicking here. You must apply to continue your insurance and pay the first annual premium within 31 days from the date coverage under your current Plan ends.
If the enrollment form and your first annual premium payment is received by Professional Benefit Consultants within 31 days from the date your current coverage ends you will be issued a new certificate for the residency coverage. You will be billed directly by Professional Benefit Consultants for future annual premiums due. Please make sure that you remit your premiums to us on a timely basis so that you do not lose this valuable coverage.
